Registration for this Congress is strictly online and via this website only.

The following contains the guidelines, instructions and deadlines concerning registration for the NHG Annual Scientific Congress 2008. Please read this section before proceeding to register online.

* Free Registration under the Special Categories will be closed on 01 October 2008. For NHG staff, NUHS staff & Students (as defined below) who register online after 01 October 2008, the online registration fee of S$100 will apply.
** After 01 November 2008, interested delegates are to register onsite at the Registration Counters, located at Level 1 Suntec Singapore International Exhibition & Convention Centre. Registration Counters will be opened on 7th & 8th November 2008. Onsite Registration Charges will apply.

Registration Guidelines

There are four (4) Sections to this online registration form. Mandatory Fields (denoted by the Asterix (*)) in all the four (4) Sections must be filled before the form can be successfully submitted. Input “NA” in the mandatory fields that are not applicable.

Payment Details

Payment must be made in Singapore Dollars (SGD) via one of the following modes:

(1)  Credit Card

(2)  Cheque / Bank Draft

All cheque / bank draft payments are to be crossed and made payable to “National Healthcare Group Pte Ltd” in Singapore Dollars (SGD).

Payment by Cheque / Bank Draft must quote the delegate name, the event attending i.e. “NHG Annual Scientific Congress 2008” and the delegate payment ID (generated upon completion of the online registration) on the back of the Cheque / Bank Draft.

(4) Payment for Onsite Registration

For onsite registration, only payments via Cash and Credit Card (Visa and MasterCard) are accepted.

Please note that onsite registration fees will apply to ALL onsite payments and bank charges will be borne by the registrant.

Congress Badge

(1) Local Delegates
  (a) Delegates who completed their registration transaction before 01 October 2008 will receive their Congress Badges prior to the Event.
  (b) Delegates who completed their registration transaction on or after 01 October 2008 will have their Congress Badges ready for pickup onsite.
(2) Foreign Delegates
  (a) Registered Foreign Delegates will have their Congress Badges ready for onsite pickup at the Registration Counter.
(3) Registration transaction would have been deemed completed upon receipt of payment fees (if applicable).
(4) Onsite pickup of Congress Badges will be at the Registration Counters located at Level 1 Suntec Singapore International Convention & Exhibition Centre between 7 – 8 November 2008.

Registration Terms and Conditions

Cancellation and Refund Policy

For NHG & NUHS Staff

Please inform the Congress Secretariat in writing before 01 October 2008 if you wish to cancel your registration. An administrative fee of SGD80.00 will be levied on the Staff who registered but did not attend the Congress. By acknowledging this section, you have authorised National Healthcare Group Pte Ltd to debit your salary for this no-show penalty fee should you fail to attend the Congress.

For NUS Students & Students from Tertiary Education Centres

Please inform the Congress Secretariat in writing before 01 October 2008 if you wish to cancel your registration. An administrative fee of SGD80.00 will be levied on the Student who registered but did not attend the Congress. By acknowledging this section, you have authorised National Healthcare Group Pte Ltd to send an invoice to your mailing address for the payment of the no-show penalty fee, which you will agree to pay upon receipt of the invoice.

For NHG GP Partners

Please inform the Congress Secretariat in writing before 01 October 2008 if you wish to cancel your registration. An administrative fee of SGD80.00 will be levied on the NHG GP Partner who registered but did not attend the Congress. By acknowledging this section, you have authorised National Healthcare Group Pte Ltd to send an invoice to your mailing address for the payment of the no-show penalty fee, which you will agree to pay upon receipt of the invoice. This cancellation policy is applicable if the NHG GP Partner received a fee waiver or a sponsorship administered via the NHG Partners Office.

For All Other Registered Delegates

Please inform the Congress Secretariat in writing before 01 October 2008 if you wish to cancel your registration.

Notification of Cancellation received:

(a) Before 01 September 2008
Full refund of prepaid fees, less an administration charge of SGD80.00.
(b) From 01 to 30 September 2008
There will be a 50% refund of prepaid fees, less an administration charge of SGD80.00
(c) From 01 October 2008
No refund will be given to the paying delegate if the delegate cancels the registration after 01 October 2008 or does not attend the Congress.

Withdrawal of Registration due to non-Shortlisted Abstracts

Registered delegates who wish to cancel their registrations due to unsuccessful review of submitted abstracts, may do so in writing to the Congress Secretariat within one month of the dated notification letter of the results of abstract review. A full refund of the prepaid fees (if applicable) will be processed. Notifications received after the one month period will be subjected to the Cancellation clauses described above.

All refunds will be processed after the Congress Event.

Return of Congress Badge

Please return the ASC2008 Congress Badges at the end of the Congress Event. The badge can be deposited into the collection bins sited at several locations within the conference venue.

A penalty fee of SGD5.00 is chargeable for non-returned badges. Payment of the penalty fee, if applicable, will follow the same procedure as that stated above under the Cancellation and Refund Policy for the various categories of delegates.

Substitution Policy

Registered delegates who are unable to attend the Congress may send an alternate i.e one-for-one substitution only. Substitution requests must be submitted in writing to the Congress Secretariat (link to email: nhgasc@nhg.com.sg) before 01 October 2008. Substitution requests received after 01 October 2008 will be subjected to an administrative charge of SGD20.00 for preparation of a new set of Congress Badge.

Letter of Invitation

Applicants requiring an official letter of invitation for the purpose of obtaining a visa or permission to attend the Congress may submit the request to the Congress Secretariat (link to email: nhgasc@nhg.com.sg)

The letter of invitation is not a commitment from the NHG ASC2008 Organisers to provide financial assistance to any applicant or their accompanying persons. Expenses incurred are the sole responsibility of the applicant.

Letter of invitation will be issued to the requesting delegate only after the registration fee (if applicable) has been paid in full.

Entry Visa Application & Validity of Passport

The registered delegate should have a valid passport, onward/return tickets, onward facilities (e.g. visas, entry permits, etc.) to their next destination and sufficient funds for stay in Singapore.

National passport holders of most countries are permitted to stay in Singapore for up to 14 days without visas. However visas may be required for visitors from other countries and participants are requested to seek clarification before travelling to Singapore. Liability & Disclaimer

The registered delegate acknowledges that he/she has no rights to lodge any claims against the NHG ASC2008 Organisers should the Congress Event be hindered or prevented to occur by any unexpected political, economic or pandemic events, or generally by force majeure, or by the non-appearance of scheduled speakers or other reasons that necessitate programme changes.

The NHG ASC2008 Organisers reserve the rights to modify the Congress Programme and Events.

With registration, the delegate accepts this provision.

Insurance

The registered delegate is advised to engage his/her own travel insurance and to extend the policy to cover personal possessions as the Congress does not cover individuals against cancellation of bookings or theft or damages to the personal belongings.
